The game's save data is contained in two files, both of which need to be moved between computers in order to get everything migrated properly.
These files are so.sol and serial.txt. I have a Mac and a Windows machine, so I'll go ahead and list the location of said files on both platforms (if anyone has any Linux info, feel free to chip in).
On the PC:

It's a warning to remind you that your health is critical, i.e. that you only have half a Heart left.
As long as you're at half a regular Heart (regardless of how many Soul/Faith Hearts you have), you will soil yourself in terror each time you enter a room.
(If you're playing as ???, this will happen when your Faith is critical instead, because he has no ...

There is no foolproof way to find the secret room (except with some of the items you can pick up, like x-ray glasses), but 99 times out of 100 you can find it with one method and a little luck.
Most secret rooms are contained in between at least 3 other rooms. All you have to do is look on your minimap for 5 rooms that are all joined in a square "U" shape ...

No, it's not possible.
The point of the game is to keep short playing sessions where death is a permanent threat. Not having the player be able to save the game both gets across this gameplay idea, and as a nice side effect it keeps players from abusing savegame files or saving mechanics (for example, save and ctrl+alt+del task manager quit the game, and ...

You can get the following pickups to cross gaps:
Ladder - Cross 1 gap spaces
Lord of the Pit - Gives you the permanent ability to fly
Hanged Man Tarot Card - Gives you the ability to fly for your current
room only, one time use
Transcendence - Like Hanged Man, only a permanent upgrade
The Bible - Gives you flight when used for your current room only

The doors are actually tied to your health. The door will only be open, if you have full health. However, the temporary HP boosts (blue hearts) can serve as a equivalent of the missing red hearts.
For example: if you're missing one red heart, and have one blue heart, the door will be open. But, if you're missing two read hearts, and only have one blue ...

According to comments on the wiki, "Everything is Terrible" makes alternate colored monsters and bosses more common. This is actually good, because alternate colored monsters tend to leave drops on kill.
According to the mechanics guide, it also makes Curses more common in Wrath of the Lamb.
Additionally, however, bosses drop one less heart pickup (heart, ...

The Binding of Isaac wiki's got a pretty comprehensive (if slightly generalized, and minorly inaccurate) list. Remember that all cards are a single use item, so once they are used they vanish.
O The Fool: Teleports the player to the starting room for that floor.
I The Magician: Gives you the use of Spoon Bender (homing shots) until you leave the current ...

The following activated items can destroy rocks:
Anarchist Cookbook - Spawns 6 bombs in random locations so there's not a 100% chance that it'll break the rock you need.
Bobs Rotten Head - Lets you throw a bomb in any of the 4 cardinal directions.
Dead Sea Scrolls - Activates as any other random activated item, so it's super unreliable.
The Doctors Remote ...

I have just managed to kill Mom, after a few previous attempts that did not go so well. What made it possible for me were two things, basically:
High health: I had 7 full hearts when I faced mom. This was partially due to luck (lots of extra health pickups/pills) and also due to me hoarding money (which allowed me to fill up on health from the shop and ...

What Snailer said is partially true. Some of the popups are tied to achievement. However, all the popups that say "... has appeared in the basement" is new content being unlocked. Mostly these are new items, and mini-bosses that you can find in the latter play throughs. Like the message you got, would mean that the item "Small rock" has been unlocked, and ...

Unfortunately, no.
In my experience, there won't always be enough keys for every door. There will be a few times when you get 2 keys, but there will be 3 locked doors, requiring you to try and decide which (if any) door you want to open.
The exception to this rule is the Skeleton Key which can be found randomly in the treasure rooms or challenge rooms ...

As Yves's comment points out, the only item that is even somewhat similar to what you describe is the Whore of Babylon. The item itself looks like this:
You can get it either by picking it up during your playthrough or by choosing to play as Eve. If you have this item and you have exactly half a heart left, it gives the message "What a horrible night to ...

The music files are located in whatever drive you installed Steam on, then this path:
\Steam\steamapps\common\the binding of isaac\The Binding of Isaac Soundtrack
Your files should be there and ready to play! There's no way to access the soundtrack through the client itself, unfortunately.
If you're on a Mac you'll find that path in ...

Strictly speaking, whenever you lose all of your regular health you will die. However, this is not always easy to do when you have soul hearts. I'll treat each of the cases you listed individually:
Blood donation machine: On levels before the womb, blood donation machines do half a heart of damage. You'll start by losing regular hearts, until you have half ...

The amount of times you'll need to use the machine varies a lot.
I've received the bag on the first attempt multiple times, but I've also once had to use the machine 32 times before the bag finally appeared.
Most of the time it's worth it to at least give it a go, as you'll generally find it around 8 attempts, which will fully heal the hearts you spent.

According to the wiki, the only item that seems to be able to raise your stats above the maximum is The Sad Onion, which doubles the firing rate no matter its current value. I have very little personal experience with that item, so I can't confirm or deny this.
The stat that I find easiest to max out is damage, as I've often managed to get quite a few ...

In Windows, the FlashAchievements.exe that ships with the game is broken for many people. I've contacted Edmund with information about a fix, not sure when that's going out but he says he's working on it after this update goes out, in the meantime the fix on this page works:

I've been experimenting for a while with these "Champions".
These are results of my observations:
different colors have the same stats. I've found 3 Champion Frowning Gapers on the same level, each one of a different color. They did double damage(1 heart instead of half) and had approximately double health. They dropped a penny, a soul heart and a bomb.

To say that the Binding of Isaac supports the use of a gamepad would be a slight exaggeration.
The latest patch for the game added control options to the game, which looks like this:
And as you can see, the game specifically tells you to use one of the many gamepad to keyboard solutions out there, specifically JoyToKey. This thread describes the ...

In the game, according to the list near the bottom of the items page on the wiki, there are
8 pills with definitely good effects:
Bad Gas: poisons surrounding enemies
Balls of Steel: gives 2 soul hearts
Full Health
Health Up
Pretty Fly: adds a close orbiting fly
Range Up
Speed Up
Tears Up
4 pills with definitely bad effects:
Health Down

From The Binding of Isaac Wikia:
Increases HP by 1 and gives 2 soul hearts.
Trivia: This is a reference to "Bandage Girl" from Super Meat Boy, another game Edmund McMillian worked on.

As duly noted, the only way to fix your face is to pick up another item which changes your face. Many items have nothing to do with your face, such as those that give you a "hat" (Cupid's Arrow), overlay (Wooden spoon) or something else entirely (Battery).
Here is a list of items that change your face (with caveats where appropriate. If you need to know how ...

In addition to the other answer, I have experimentally found that the lucky foot increases the chances of winning at the shell game in the arcade. The wiki says that the probability of winning on any single run is 33%, but when playing as Cain I have recorded a greater than 50% win rate (so far it seems to be 66%).
Edit: according to discussion on the item ...

This is not a statue, but Satan, and you have found what's referred to as a "devil room". These two particular items are Sister Maggy (on the left), a familiar that follows you and shoots blood tears in the same direction that you do, and The Mark, which increases your damage by 2 points.
Be aware that the cost depicted in hearts will be deducted from your ...

My original answer stated that this would not work, but I have discovered that is incorrect. Poo piles that Larry Jr drops will in fact drop items. So far I have only observed hearts and soul hearts, but I would assume that the drop rate is the same as that of any other poo piles. The piles will drop items if the player pops them or if Larry pops them.
It ...

For one thing, as you have noticed, all monsters will do a full heart of damage, even those that did less in previous dungeons (unless you have the Wafer, which grants damage resistance). This applies to Blood Donation Machines as well.
Some special rooms (the Shop, and the Item Room) aren't present here, but you can still get Arcades and Gauntlet Rooms. ...

Here's three things I noticed about their pattern:
They react to your movement. If you stand perfectly still, they will stay down.
They will pop up only to attack, which they generally will not do if they don't have a line of sight.
Bombarding them with projectiles while they're down is useless. If you hit them in this state, they will stay down for a ...

The Skatole is the only significant item you can acquire from the skull game. After getting the Skatole, it's an endless loop of coins, hearts, keys, and bombs. Also the occasional fly, which won't be an issue if you have the Skatole.
From the Binding of Isaac Wikia:
Shell Game: Bet 1 coin and a bomb, heart, key, coin, or the grand prize of Skatole is ...
