They unlock characters. You unlock one character after 10 bandages and then another for every 20 that you collect beyond that up to 90 and then 10 more gets you the final unlock. The precise characters gained will vary depending on whether you are playing the XBOX version or the PC version.

Super Meat Boy is easier with a gamepad only if gamepad is the input device you are more at ease with generally speaking, that's all. If you continuously die, Band-Aid Girl should eventually become glitched. While she is glitched, standing next to her should teleport you to the negative level. And you are correct, there are negative levels for Worlds 1 - 6. Here is a video of somebody unlocking the first negative level.

There are multiple glitches with the completion percentages. You can find quite a few references to it if you do some searching a bit for "Super Meat Boy" and numbers like "109", "96", etc. Here's an example topic on a forum. Contrary to the above answer, you can make it out alive even if you land on the side of the building (I just did). Just get below the buzzsaw, and jump directly to the right, so you end up above the repulsor. The repulsor will give you enough vertical push to make it over the buzzsaw.

Every time you die, there's a certain chance that Bandage Girl becomes glitchy (in that certain chapter - light or dark world doesn't matter). Save her in this glitchy state to unlock the glitch level of that chapter. Each one brings you an achievement when you beat it.
