When you pass through that door, in either direction, you take half a heart of damage. On the other side you will usually find red chests, which usually have better stuff than normal chests (like two pills or an item), but sometimes have bad stuff like enemies or troll bombs that follow you. When you complete the Womb by killing It Lives, you can choose where to go. There will be available a beam of light, that brings you to the Cathedral and Isaac, and the trapdoor, that brings you to Sheol and Satan. I tested this actually. Took a while to grind so the eternal heart card would spawn on basement 1. Everything adds up. Just because you can only see up to 12 containers (of any kind) doesnt mean that is the limit. I had managed to get up to 14 heart containers. In addition to the flight ability that you observed, Spirit of the Night also makes your shots spectral (i.e. they pass through anything but enemies and rarely interact with environment items that you can shoot, such as fire and poop). The white fly is an eternal fly, and will hurt you if it touches you. Additionally, if the poop is destroyed, the eternal fly will be reincarnated as a tougher red fly. In the DLC, there is always a chance of entering alternate versions of levels: Basement = Cellar Caves = Catacombs Depths = Necropolis Womb = Utero Sheol = Cathedral At the end of Womb/Utero, you have a choice of entering Sheol or Cathedral. All the other floors are random.

I've done many runs since the DLC came out, with different characters, never doing a challenge. Every time I killed It Lives, I've found both the trapdoor and the beam of light. Your guesses are correct. The puzzle is completed by beating the Cathedral six times. Here it is after my second victory against Isaac: Completing the puzzle unlocks the Polaroid trinket, an item that shields Isaac when brought to 1/2 heart, but that also
